Interactive Web Forms

Web forms are used to collect information from people viewing your web pages. Forms can include any of the following:

  • Single-line text fields (can be marked as required)
  • Multi-line text fields (can be marked as required)
  • Check boxes (choose zero or more)
  • Radio buttons (choose one)
  • Multi-select
  • Drop-down menu

Form information can be sent as an email and/or stored in a spreadsheet.

Notes:

  • There is currently a limit of one form (including polls) per web page
  • Form assets are locked to a site, so will not be available on other sites
  • If you are using a form to collect sensitive information [docx] such as Date of Birth or Social Security Numbers, please open an iSupport ticket to secure your form via HTTPS

Creating a Form

To create a form:

  1. Go to Content -> Assets
  2. Click the +new button at the top right of the page
  3. Choose Managed Form
  4. Give your asset a name and description
  5. Add meaningful tags to help you find the asset later (optional)
  6. Enter the Success Message (this will be displayed when the form has been successfully submitted)
  7. Enter the Failure Message (this will be displayed if there are problems with the form submission)
  8. Leave the Save Results in Database box checked if you would like all results available as a spreadsheet
  9. If you want to have an email sent every time a form is submitted, click the New Message button
    1. Fill in the To, From, Subject, and Body fields
    2. You can include data from the form by entering the name of a form element inside double curly braces. For example, you might have {{email}} in the From field if you have a field for user's email address, or {{comments}} in the Body field, etc.
  10. Add the desired elements
    1. The initial value entered for Label will be used as the Name of the element
    2. The label can be changed later, but the name will not change
    3. Check the Required box if this field can not be empty
      Note: Please indicate in the label that the field is required. There is currently no automated way of indicating which fields are required.
    4. Choose Validation if required. Users will be visually notified about which fields are not valid so they can fix them before re-submitting the form. Validation options are:
      • Valid email address
        There is a known issue with capital letters in email addresses causing them to not validate properly. It is currently being investigated. In the interim, use the Regular Expression validation option and enter the Email Address validation code below into the field.
      • Minimum length
      • Regular expression (Advanced users only)
        Email Address validation code: 
        [a-zA-Z0-9._%+-]+@[a-zA-Z0-9.-]+\.[a-zA-Z]{2,4}>
  11. Add as many new elements as you need
  12. Save and publish
  13. Add to the page where you want the form

Viewing Form Data

  1. Go to Content -> Assets
  2. Find the asset in the list
  3. Check out and edit the assset
  4. Click View results link at the upper right corner
  5. If you want to download the information, use the Download as a CSV link.
